The artists design a special occasion cover which can be attached to a 12" x 15" sign-in book (book is sold separately).   The art is  in colorful 3D lamination featuring the person you are honoring.

Ordering Information: We need a clear photo via email (ideally a wallet-size color school or passport headshot) and a completed questionnaire (PDF, Get PDF Reader Here).
